The POTENTIAL INHIBITION OF THE ANESTHETIC REVERSAL AGENT - PHENTOLAMINE MESYLATE - IN DENTISTRY

Objective: This study aimed to analyze the inhibition capacity of phentolamine mesylate, a dental anesthetic reversal agent, on alpha-1 and alpha-2 adrenergic receptors in blood vessels, seeking to shorten the duration of local anesthesia and restore function in the affected area through vasodilation. Methodology: An integrative literature review was conducted, with a bibliographic search in the PubMed database in July 2024. Specific search strategies were used, resulting in 124 articles initially. After applying inclusion (RCTs, full texts, all languages, published between 2019 and 2024) and exclusion criteria (duplicates, laboratory studies, systematic reviews, and irrelevant articles), 4 articles were selected for analysis. Results: The analyzed studies demonstrated that phentolamine mesylate is effective in reducing the duration of local anesthesia, especially when combined with 0.5% bupivacaine and 1:200,000 epinephrine, reducing anesthesia time by an average of 2 hours and 17 minutes. However, adverse effects such as injection site pain, tachycardia, and increased postoperative pain were observed in some cases. Phentolamine was shown to be safe for use in pediatric patients and those with special needs, although it is contraindicated in cases of hypersensitivity or peptic ulcer. Conclusion: Phentolamine mesylate is an effective reversal agent for shortening the duration of local anesthesia in dentistry, promoting vasodilation and rapid restoration of oral function. Its safe use requires detailed pharmacological knowledge, considering contraindications and potential adverse effects. Despite the risks, its application is recommended to reduce postoperative complications and shorten recovery time.
